Religious School Registration

I give my child permission to attend the education programs at Congregation Beth Israel. I hereby release and hold harmless CBI and its respective employees, of and from any and all present and future claims of any kind of nature arising from my child's attendance and CBI religious School. I understand that students must remain on CBI grounds from the time that they arrive through the end of their scheduled program unless they are part of an authorized chaperone activity. I give permission for my child to leave the grounds for education programs under the supervision of the program staff.

Registration is open to all members in good standing. This means all prior tuition charges, fees and membership dues have been discussed, paid in full or arrangements have been made with the CBI office. Please call (503) 222-1069 before registering if this is not the case.

If interested in scholarship, please continue to fill out this form. Scholarship requests must be discussed with the CBI office by September 1st, 2021, as scholarship funds are limited.

There will be no refund to families who choose to withdraw their child, and tuition will not be prorated for families who enroll after the academic year has begun.

Payment is due in full by December 31, 2021 unless monthly payment plan is chosen and discussed with CBI.
